Which Rhinoplasty for your case?

Dorsal rhinoplasty (giba):

In my routine, removing the nasal hump is one of the reasons common causes that lead patients to seek rhinoplasty. The nasal hump is a bony and cartilaginous prominence (“lump” or “bump”) on the bridge of the nose that can cause aesthetic and even functional discomfort. Rhinoplasty for the treatment of this "bone callus" it is not just a "slushie". We have to work on the nasal bones, cartilage and then rebuild the nasal dorsum, in addition to adjusting the tip of the nose to the new dorsum. The technique of preservation has the advantage of making the surface of the nasal dorsum more regular, without the need for reconstruction and that we do in selected cases (good for thin skin!). The use of more modern instruments such as ultrasonic rhinoplasty and drills diamonds brought much greater precision in the structured technique and we used ever!

Tip Globose rhinoplasty:

The perfect nose tip should match the rest of the nose and face, and skin types and cartilage are fundamental to choose the best techniques to work the tip of the nose. Generally, to better define the tip, Dr. Felipe uses a combination of modern surgical techniques (structured rhinoplasty with grafts and preservation for avoid pinching), seeking to offer a more defined and harmonious tip, but also that doesn't crumble with time! In patients with more oily skin it may be necessary to associate a skin treatment to improve the result. And it's always important remember that this is the region that takes the longest to deflate!

Alectomy (Narrowing of the Nasal Base):

Alectomy is surgery performed to reduce the width of the nostrils. There are different types of alectomy and although it is one procedure that may seem simple, requires a lot of experience because it can take important sequelae with unwanted scars and nasal obstruction. She is always performed at the end of the rhinoplasty when the entire nose is ready and should not be aggressive. It is hardly an isolated procedure as I have seen more recently in the office and performed by non-physicians. During the consultation, I try to explain everything to the patient. understand its real indications and limits well.

Rhinoplasty in the Afrodescendant Nose:

Although Brazil is remarkable for the large miscegenation and also why I think we are such a beautiful people, there are some characteristics that are more common in this type of nose and that can be worked on without lose this essence. The skin tends to be thick and bushy, the top of the nose (called the nasal root) tends to be lower, the projection (height) of the tip is less with a less defined edge. This type of nose usually has less cartilage and the nostrils are wider. Often, the nostrils dilate too much when smiling. In these cases, we generally seek to narrow the base of the nose and the bony part, better define and project the tip, in some cases even increasing the height of the nose, but always without losing the patient characteristics!

Functional rhinoplasty

It is indicated when it is necessary to work on the external structure of the nose with the aim of improving breathing only, with no primary aesthetic purpose. To the nasal valves are the narrowest regions of the nose and most resistant to flow airway and when this region loses support, it can collapse during inspiration. This weakening or insufficiency can be caused by a surgery or previous aesthetic procedure, accidents and even aging itself. In in many cases we associate septoplasty and turbinectomy to improve the results.

Augmentation rhinoplasty:

This rhinoplasty requires special skill and experience due to the need for grafts to enlarge the nose, either in the “oriental” type nose, after trauma or previous rhinoplasty (“saddle nose”). Patients tend to complain of nose short, broad ends, wide nostrils and low back. The objective is always to refine the nose, without lose ethnic characteristics (in the case of the nose of oriental origin). Generally we need to increase the height of the back, refine the tip of the nose and narrow the base a little nasal. We have experience in being able to use your own cartilage, whether from the septum (nose), ears or even the rib. Although it may seem strange to use cartilage from other parts of the body, the truth is that the septum alone may not be enough. AND It is important for the patient to know that these incisions can be well hidden.

Revision Rhinoplasty

It is the surgery performed on patients who have already undergone a or more nose surgeries without satisfactory aesthetic and functional results. although in many cases may be simple in order to correct only some detail or irregularity, it is usually much more complex than primary rhinoplasty (the first surgery on the nose). In these cases, the nasal anatomy is altered, requiring a high degree of knowledge technical, scientific and artistic of the surgeon to restore this anatomy. It is important that the surgeon has extensive experience in surgical revisions as it usually requires greater use of grafts (which can be ear or rib) and a greater concern with the skin quality due to previous scars. Procedures are often more time consuming as well as the healing time for the definitive result.

Rhinoplasty on the Crooked Nose

The nose is the center point of the face and important for the face balance. This rhinoplasty is indicated for both the aesthetic and functional part. (usually with associated septoplasty). This defect can be congenital or due an accident suffered in childhood or adulthood. The diagnosis of a crooked nose is made with based on clinical exams, photo analysis and computed tomography. It is fundamental evaluate the face as a whole through analysis parameters and facial measurements to know what defects exactly do we need to correct, but also why the crooked nose may be associated with a very asymmetrical face. I think patients are important understand that the goal is always to improve and not the perfectly straight or symmetrical. Incidentally, few people have a 100% symmetrical nose and even so they can be considered beautiful!

I am a doctor graduated from the Faculty of Medicine of the University of São Paulo – FMUSP. (1996-2001) and completed my specialization in Otorhinolaryngology (Medical Residency) at Hospital das Clínicas da FMUSP (2002-2004). I was invited and acted as Preceptor Physician for residents of the Department of Otorhinolaryngology at HCFMUSP (2005).

In 2006, I had the opportunity to carry out specialization in the most renowned advanced nasal endoscopic surgery centers in the world (University of Graz, Austria, with Prof. Stammberger and University of Pittsburgh, USA). In 2008 I started my Doctoral thesis at FMUSP, defended in 2011 on advanced nose surgery, which yielded international publications.

During this period I was also approved in the medical degree equivalence tests in the USA (USMLE 1, 2 and 3) and I did internships in facial plastic surgery services, focused on rhinoplasty, at the University of Sacramento (with Dr. Sykes), Chicago (with Dr. Toriumi) and Portland (with Dr. Wang).

Seeking to expand my knowledge in structured and plastic rhinoplasty of the face, I completed a postgraduate course with Dr. Jurado in São Paulo (2011-2012). With the resurgence of Preservation Rhinoplasty, I interned with Dr. Goksel in Istanbul, Turkey (2019).

Since the end of my training, I have dedicated myself to my private patients and academic life, participating as a speaker in several courses and congresses in Brazil and abroad, with several publications in medical journals and book chapters.

I hold a specialist title in Otorhinolaryngology and Cervico-Facial Surgery by the ABORL-CCF and I am a member of the Brazilian Academy of Facial Plastic Surgery (ABCPF) where I was a member of the board between 2012 and 2016, of the Rinoplasty Society of Europe (RSE), of International Federation of Facial Plastic Surgery Societies (IFFPSS), member of the Brazilian Society of Rhinoplasty (SBR) and meritorious member of the Otorhinolaryngology Foundation (FORL).

Septoplasty

A deviated septum occurs when the wall that divides the nostrils is displaced. There are several types of deviation, which may be cartilaginous and/or osseous. They can be of hereditary origin or caused by traumas from birth to life
adult. Symptoms can range from complete blockage of one nostril, which can severely impair the patient’s ability to breathe. I always believe it is essential to evaluate the septum of all patients who are candidates for rhinoplasty, even in cases of cosmetic rhinoplasty. When necessary, we indicate your
correction, which can and should be performed along with rhinoplasty (by the way, always the ideal time!).

Surgery of the nasal turbinates (turbinectomy):

The turbinate (or nasal concha) is a spongy organ located on the lateral wall of the nose. Its hypertrophy (or increase) occurs when there is chronic inflammation of the nose (generally in patients with rhinitis, sinusitis and septal deviation), which can cause respiratory difficulties in addition to chronic infections. In some cases, it may be necessary to associate turbinectomy (removal of the nasal concha) with functional rhinoplasty to help with breathing and rhinitis control. Dr. Felipe always uses the minimally invasive and conservative technique for a quick recovery and avoid complications.

Adenoidectomy

It is the surgical procedure responsible for removing the lymphoid tissue located in the upper part of the nasopharynx (communication between the nose and the mouth). It is popularly known as spongy meat and is more common in childhood. In some patients, it must be associated with tonsillectomy, which may also be enlarged, making breathing difficult or causing snoring, sleep apnea and infections. I always try to evaluate this functional aspect in candidates for rhinoplasty as well.

Sinusectomy

It is a surgical procedure indicated for patients with recurrent sinusitis, chronic sinusitis, nasal polyposis and some cases of nasal tumors. It is performed when there is little result with clinical/drug treatment. The purpose of this functional surgery is to open, drain and aerate the paranasal sinuses, seeking to restore function and thereby allowing better access to topical medications in the postoperative period. It can be associated with rhinoplasty. It is a procedure that brings great relief to the patient and that we perform endoscopically.

Otoplasty

Otoplasty for correction of protruding ears is another very rewarding aesthetic surgery in our clinical practice. It can be done with local or general anesthesia. When done at the right time, it can have a huge effect on self-esteem and social development. It can be done in children from 7 years old as well as in adults! I think it is essential to evaluate together with the parents so that we can decide together the best moment to operate. It can be associated with rhinoplasty.

Mentoplasty

It is the surgery that has the purpose of increasing or remodeling the chin, aiming to improve facial harmony. This procedure is usually performed after complete facial growth, which usually occurs after the age of 16 – 18 in
teenagers. For chin augmentation, it is possible to use hyaluronic acid filling or fat grafting (chin fat grafting), implants (prostheses) of silicone or porous polyethylene (Porex or Medpor), or it can be done with bone advancement. I think it’s important to talk to each patient about the advantages and disadvantages of each technique. Mentum surgery is commonly associated with rhinoplasty, providing a more harmonious balance of the face and more satisfactory results. In some patients, the small chin contributes a lot to the impression of the large nose and needs to be evaluated!
